After patch to RHEL5.5 Server will not boot

Issue

  • after patch to RHEL5.5 Server will not boot. 
  • fails to find root Volume group.
  • Multipath is not recognizing the devices on boot.

  • Following message on boot:

    Creating multipath devices
No devices found

Environment

  • Red Hat Enterprise Linux 5.5
  • lpfc card with lpfc_use_msi=2 specified as an option to lpfc.ko
