  • Qlogic HBA loop state is DEAD

    grep -i "loop state" /proc/scsi/qla2xxx/0
Host adapter:loop state = <DEAD>, flags = 0x5a03

grep -i "loop state" /proc/scsi/qla2xxx/1
Host adapter:loop state = <READY>, flags = 0x45a03

Environment

  • Red Hat Enterprise Linux (RHEL) 4 Update 8

  • QLogic PCI to Fibre Channel Host Adapter for QMH2462
    Firmware version 4.04.09 [IP] [84XX] , Driver version 8.02.09.01.04.09-d
