RHEL5: IBM pSseries server with Broadcom NIC crashes after "iommu_alloc failed" messages in log
Issue
An IBM POWER based machine crashes or hangs shortly after displaying one or more messages in the log similar to:
iommu_alloc failed, tbl c000000001ab8280 vaddr c00000005e660000 npages 1
iommu_alloc failed, tbl c000000001ab8280 vaddr c000000084170000 npages 4
Environment
- Red Hat Enterprise Linux 5.8
- IBM POWER hardware
- Broadcom "Tigon 3" network interface
