kernel: qla2xxx 0000:13:00.0: Unknown status detected 0x13-0x0 observed on Red Hat Enterprise Linux servers

Issue

  • I am getting following message in log files:
    kernel: qla2500 0000:13:00.0: Unknown status detected 0x13-0x0
  • Our storage intermittently becomes unavailable with the following error message:
    kernel: qla2xxx 0000:0c:00.0: Unknown status detected 0x13-0x0.
  • We are facing issue with one of the most critical server of our Env. where I/O errors are reported on the disk presented to the server for the Oracle DB running over it. The Database on the server once crashed when the IO error was reported while on the other times the jobs running on the server got error-ed out/aborted.

Environment

  • Red Hat Enterprise Linux (RHEL)
    • Red Hat Enterprise Linux 5 (RHEL5)
      • Issue has been observed on kernel-2.6.18-128.el5, but may exist on other versions.
    • Red Hat Enterprise Linux 6 (RHEL6)
  • QLogic driver (qla2xxx) provided by Red Hat Enterprise Linux kernel.
  • Issue has been observed on IBM DS8300 SAN (product 2107900), but it is unknown if other Vendor and models are affected.
  • The following error message is observed in /var/log/messages:
