Four port Qlogic adapter may cause EEH error during kdump

Issue

  • During a crash a 4-port Qlogic adapter may cause an EEH in the kdump
    environment due to a race condition between the embedded switch and
    the two controllers found in 4 port adapters.

  • The following errors may be displayed in the console:

EEH: Detected PCI bus error on PHB#23-PE#10000
EEH: This PCI device has failed 1 times in the last hour and will be permanently disabled after 5 failures.
EEH: Notify device drivers to shutdown
EEH: Beginning: 'error_detected(IO frozen)'
EEH: PE#10000 (PCI 0023:01:00.0): no driver
EEH: PE#10000 (PCI 0023:02:02.0): no driver
