Receiving "device-mapper: create ioctl failed: Device or resource busy" after adding storage to a RHEL5 server

Issue

  • Adding additional disk to a server, going from 123 to 152 luns through a dual-ported HBA.  Once new storage was seen on the server and the disks partitioned, a reboot causes errors from device-mappers after udev rules are started.
  • Allocated ~150 LUNs

  • Red Hat Enterprise Linux 5.4 or earlier
  • Red Hat Enterprise Linux 5.9
  • kpartx-0.4.7-30 or earlier
