Dialog Values are Not Populating Correctly for Service Reconfigures

Issue

After upgrading from CloudForms 4.5 (CFME 5.8) to CloudForms 4.6 (CFME 5.9), dialog values for service reconfigures are not populating with the expected values. Many of the fields are showing up as blank.

Environment

CloudForms 4.6.0, 4.6.1, 4.6.2 (CFME 5.9.0, 5.9.1, 5.9.2)
