RHEV storage domains showing with 'Unknown' status after temporary loss of connection to NFS storage

Solution Verified - Updated -

Issue

We had an incident wherein the network hardware connecting to our NFS-based RHEV cluster was disconnected.  This caused all the virtual machines to die, as expected.  Upon things coming back, we rebooted all the RHEVH nodes but the NFS storage is not coming back online.

In the RHEVM GUI, the status of all the storage domains is showing as 'Unknown'.  The RHEVH nodes are appearing as active and one has taken the role of SPM however the SPM status isn't moving to another node when we put the SPM node into maintenance mode.  If we try to attach the master storage, a blank box comes up that says "Attach Storage" but the only option is to close the box.
