RHEV : Operation Add-Disk failed to complete with error "can't map PV /dev/mpath/mpath1"

RHEV operation add-disk fails when adding disk to existing RHEL server in stopped state. When we try to add the disk we can see the existing disks and the new disk being created. It shows nothing in the capacity available and then approximately 30 seconds later it errors. Tried in other servers under RHEL control on the same hypervisors, they fail. Restarting hypervisors did not clear the fault. Tried in other servers assigned to alternative hypervisor, it worked fine.
