Virtual Machine fails to boot with error : "FATAL: NO bootable device". How can I fix it?

Starting a virtual machine from virt-manager fails with the following error message on the VNC console:
FATAL: NO bootable device
Environment
- Red Hat Enterprise Linux 5.x
- Red Hat Enterprise Linux 6.3 and earlier
- Red Hat Enterprise Virtualization (RHEV) 3.0, 3.1
