[RHEL] Unable to mount a filesystem, the mount command claims the volume is already mounted or the mount point is busy.
Issue

When attempting to mount a filesystem I see
[root] # mount /dev/mapper/VG01_lv_app /var/app mount: /dev/mapper/VG01_lv_app already mounted or /var/app busy
Environment
- Red Hat Enterprise Linux 6 (all versions)
