"Error Uploading Submission" is faced when the form is submitted with Camera field or Signature field

Issue

• Error Uploading Submission is faced when the form is submitted with Camera field or Signature field
• Error HTTP code 500 is shown when is trying to save the data using a form made via the Incident Report template
• Camera access not working
  The following is an example of this request made which is returning the HTTP code 500.
