Why is access to my Veritas VXFS filesystems is hanging on Red Hat Enterprise Linux 5?

Issue

Access to external storage hanging with vxfs, although the rest of the system is still responsive.
Processes trying to access filesystems is in "D" state
LS in the filesystem hangs the terminal
