pam_mkhomedir Fails to Create Home Directory When User Logs in Using SSH

Issue

When a user logs in to a machine for the first time via ssh, pam_mkhomedir may be used to automatically create the user's home directory. If the user's home directory is not automatically created when logging in via ssh there may be a couple different reasons, noted in the resolution section.
