pacct log file grows constantly in Red Hat Enterprise Linux 6

Issue

• We seem to have some weird behavior lately with the process accounting:

  -rw------- 1 root root  95M Feb  3 03:48 pacct-20150202.gz
  -rw------- 1 root root 270M Feb  5 03:44 pacct-20150204.gz
  -rw------- 1 root root 3.0G Feb  5 03:45 pacct-20150205
  

• The file is growing practically all the time in our RHEL servers with Oracle RAC.

• On the system, it seems that dmsetup and basename were executed repeatedly.
• The only change that has happened recently in this system is the installation of the glibc patch.
