LVM metadata corruption on RHEL standalone/cluster systems with HP Operations Manager agent

Issue

  • checksum error while executing pvs, lvs commands on RHCS
  • Cluster service failing due to corrupted LVM metadata on fail-over in a cluster
  • After re-sizing logical volume in Red Hat fail-over cluster, logical volume becomes unavailbale and cluster is failing
  • We are observing LVM device metadata corruption after seemingly innocuous commands like vgs, lvs, vgscan, etc are executed

Environment

  • Red Hat Enterprise Linux (RHEL) 5, 6, and 7
  • HP Operations Manager installed and running - specifically, midaemon is running
