Issue
Server panics after upgrade from RHEL 6.1 to 6.2 with the following lead up messages:
dracut Warning: LVM rootvg/lv_root not found
dracut Warning: Boot has failed. To debug this message add "rdshell" to the command line
Kernel panic - not syncing: Attempted to kill init!
