Why doesn't my volume group activate at boot, but can be activated manually?

Issue

Volume Group comprised of multiple PV's attached via multipath from does not activate at boot
After booting, the volume can be activated with vgchange -ay <VG>

Environment

Red Hat Enterprise Linux 5
SAN storage with paths aggregated by device-mapper-multipath
LVM with Physical Volumes on multiple SAN LUNs
