Infinite loop on all new presented SAN LUNs

Issue

Since the addition of a new SAN disk, followed by a reboot, the
system seems to have an infinite loop on all SAN LUNs with these
messages:

[sdXX]: Sense Key : No Sense [current]
[sdXX] Add. Sense: No additional sense information.

If the host boot the latest system rescue livecd, the same loop happens.
