incorrect LDAP search results "ERR - task_index_thread - Index failed" and "Requested to index entrydn, but nsslapd-subtree-rename-switch is on"

Issue

Scenario example:
Lately we have been experiencing odd behavior with indexes related to automountmaps.
LDAP queries to automountMapName= have stopped working until we re-index the attribute.
We recently migrated from RHDS-9 and RHDS-10 to RHDS-12 and found that we needed to add indexes for automountinformation, automountKey and automountMapName.
But after we implemented the indexes we have found that for one of our consumers and our two supplier systems we've had to re-index the automountMapName attribute every so often so queries would work again.
LDAP search results would work for awhile and then it would not return any entries.

The following LDAP errors log file events were noticed:

[17/Apr/2025:11:33:23.595346101 -0700] - WARN - bdb_db2index - userroot: Requested to index entrydn, but nsslapd-subtree-rename-switch is on
[17/Apr/2025:11:33:23.596208253 -0700] - ERR - task_index_thread - Index failed (error -1)

Environment

RHDS-12 on RHEL-9 migrated from previous RHDS major versions
