VMWare IPI install does not create nodes in OpenShift 4

Issue

Using solution How to deploy OpenShift 4 with a custom domain for applications,
 ran the openshift-install create cluster --dir=/opt/openshift-install --log-level=debug, but the nodes were not created, only the masters.
When looking through VMware, the workers' VMs are not created, even with the machine objects created for the workers.
