Upgrade from rhel 6.10 to 7.6 fails - the kernel panics
Issue

After upgrade from RHEL 6.10 to RHEL 7.6 and it appears to complete successfully.
- Once upgrade complete, the system reboots automatically.

Then kernel panics, because of missing entry for initramfs in grub
