Issue
- qemu processes are hung in D state after to underlying storage problem
- It is impossible to restart the libvirt vm from Openstack or virsh.
Environment
- Red Hat OpenStack Platform
- instance using cinder block storage from FC/iSCSI
- All path of a LUN failed belonging to the instance
