Reboot after kernel upgrade led to a crash boot on SAN

Issue

• After a kernel upgrade from '2.6.18-194.el5.x86_64' to '2.6.18-194.11.4.el5.x86_64', the reboot into the new kernel led to a kernel panic with the message

  Unable to access resume device (/dev/VolGroup00/LogVol01)
  mount: could not find filesystem '/dev/root'
  setuproot: moving /dev failed: No such file or directory
  setuproot: error mounting /proc: No such file or directory
  setuproot: error mounting /sys: No such file or directory
  switchroot: mount failed: No such file or directory
  Kernel panic -- not syncing: Attempted to kill init!
  

• Booting into the earlier kernel (2.6.18-194.el5.x86_64) did not have any issue.
