Red Hat Enterprise Linux 6.1 から 6.4 にアップグレードすると、mount: /dev/sda1 already mounted or /sysroot busy メッセージとともにカーネルがパニックが発生する

Issue

• システムを Red Hat Enterprise Linux 6.1 から 6.4 にアップグレードします。
• 新しいカーネル 2.6.32-358.el6.x86_64 で起動するとシステムがパニックになり以下のエラーメッセージが表示されます。
