ssh issue

Need help for this case, Some time while trying to connect through ssh connection get closed

some time error come like this
ssh_exchange_identification: Connection closed by remote host

some time like this error happening
srpuser@10.147.135.12's password:

then i execute this command to debug then i got this kind of output
please help me for further solutions
