Error: Rhel upgrade 6 to 7

Hi

I have been having the following issue trying to upgrade my rhel 6 to rhel 7
I have been following the guide :

I run the following command :
redhat-upgrade-tool --iso /var/isos/rhel7dvd.iso
and the reboot

It then goes through the mostion till it load the "ldconfig" and then the errors begin to follow:

Not sure what else can i try, been through the motions with google but to no avail.

Any assistance would be appriciated.
