System may panic with Kernel oops while using 6.1.z and the lpfc driver

Issue:

• During FCP service stop/start tests, a kernel crash was hit at scsi_request_fn on a RHEL 6.1.z Emulex FC host.
• The event has not been replicated on RHEL6.2, and is not easily reproduced on 6.1.z.

Environment:

• RHEL6.1.z - kernel-2.6.32-131.0.15.el6
• Emulex LPe1205-HP FC HBA
• Other controllers may be affected
