How do I configure iSCSI installation and booting (open-iscsi) in Red Hat Enterprise Linux 5.1 and higher?

iSCSI installation and boot was originally introduced in Red Hat Enterprise Linux (RHEL) 5 as a Technology Preview. This feature is fully supported as of RHEL 5.1 and RHEL 6. This capability has three configurations depending on whether the system is:
