How do I recover from a lost software RAID device in Red Hat Enterprise Linux 3 or 4?

Sometimes, unexpected things happen to software RAID devices. For example, a RAID 5 configured with three drives should be able to sustain even if one of the disks fail. In some cases the RAID device no longer recovers. One of the first things that can be done is to try reassembling it using automatic detection methods. For example,

mdadm -Ac partitions /dev/md0 -m dev
