How do I find which SCSI device is connected to which SCSI interface?

Updated -

To find out which SCSI device is connected to which SCSI interface, get the output of the following command:

# cat /proc/scsi/scsi

For example the output is like this:

Host: scsi0 Channel: 00 Id: 00 Lun: 00
Vendor: SEAGATE Model: ST336807LW Rev: 0C01
Type: Direct-Access ANSI SCSI revision: 03
Host: scsi0 Channel: 00 Id: 03 Lun: 00
Vendor: HP Model:...
